Program Outline

The Coordinated Veterans' Care Program (CVC) is a new Department of Veterans' Affairs program aimed at better managing and coordinating primary and community care for Gold Card holders who are most at risk of being admitted or readmitted to hospital. The CVC Program is focussed on providing additional suppport for veterans with one or more chronic diseases or conditions and who have complex care needs. The Veteran must not be registered with another similar program (ie. Community Aged Care Packages).

The CVC Program is initally targeting patients with congestive heart failure, coronary artery disease, pneumonia, chronic obstructive pulmonary disease and diabetes which research has shown can be responsible for high rates of admission/readmission to hospital.

Identifying potential participants

  • DVA through BUPA Health Dialog via mail out to patients and GPs
  • Patient identified by GP
  • Gold Card holder requests to be enrolled in the program

By participating in the program GPs are able to claim through Medicare a Initial Incentive Payment for enrolling a participant in the program and Quarterly Care Payments for ongoing care.
