The focus of the role of the Aboriginal and Torres Strait Islander Outreach worker is to provide community liaison, establishing links with local Indigenous communities to encourage and support the increased use of primary health care services including Indigenous Health Checks.

In addition the Outreach Worker will provide feedback in relation to problems that impact on Aboriginal and Torres Strait Islander access across the primary health care sector.

The Aboriginal and Torres Strait Islander Outreach Worker also provides transport services (transport for urban areas only at this stage), practical assistance and support to patients to:

  • Attend appointments with their GP, practice nurse, specialists and care coordination
  • Attend appointments with other primary health care providers
  • Attend appointments for diagnostic tests.

Who can access the Outreach Worker?

Any person identifying as being of Aboriginal or Torres Strait Islander origin.

Is there a cost?

No, this service is offered free of charge.

How do patients access this service?

The service is available through referral from mainstream primary health care services and service providers and community service organisations.
